To be eligibile you had to pay Self Emplyment Tax in 2020 and/or 2021
Instructions on filling out the SETC Form:
Step 1: right click on this link: SETC Form
Step 2: click "open in new tab" (shown below). On the new tab complete the form. Keep this tab open to refer to these instructions.
Step 3: scroll to the bottom and click "Continue" (shown below)
Step 4: fill out questions 1 - 12
Question 11a your name will autofill when you (the perparer) sign after question 12
be sure to enter the correct email address. Subsequent steps will require communication with the CPA.
